At Your Aging Navigator, our goal is to make that starting point easier. Here’s what you can expect when you contact us for aging guidance and planning support.
Step One: The Initial Call or Inquiry
The first step is often a phone call or email. There’s no need to have everything figured out before reaching out. Many families contact us during times of change, stress, or uncertainty.

This first connection is a chance to share what’s going on. We’ll ask a few questions to understand your situation and determine the right next step—usually a more in-depth conversation we call the Discovery Session.
Step Two: The Discovery Session (2–3 Hours)
This session is a structured, yet personal discussion. It typically lasts two to three hours and allows us to fully understand the care landscape you're managing.
During this session, we explore:
Current care needs and challenges
Who is involved in caregiving or decision-making
Goals, values, and preferences for the future
Legal, financial, housing, and health care considerations
This process helps families gain clarity and see how all the pieces connect. For many, it’s the first time these topics are discussed openly in one place.
Step Three: Your Personalized Aging Roadmap
After the Discovery Session, we create a customized aging roadmap based on your unique situation. This document outlines:
Key takeaways from our conversation
Areas that need immediate attention
Recommendations for next steps
High-level guidance on what to expect moving forward
The roadmap is designed to support thoughtful aging planning. It helps families make informed decisions, reduce uncertainty, and understand where support may be helpful.
Step Four: Ongoing Support and Implementation
If ongoing support is needed, we offer continued guidance as your plan is put into action. This may include:

>>Active navigation – during care transitions like hospitalizations or housing moves
>>Long-term planning – ongoing check-ins and coordination as needs evolve
As part of this step, we may introduce families to trusted professionals—such as elder law attorneys, home care providers, financial advisors, or housing specialists. These referrals are personalized and made only when appropriate, based on your goals and readiness.
